Insurance Information

Premiere Physical Therapy accepts several major insurance plans. Below is a list of the plans for which we provide service most often. Benefits can vary within each plan so we encourage you to call us with coverage questions at 217-744-8888.

If your insurance plan does not appear on this list, then we still encourage you to call us. We will personally call your insurance company and check your physical therapy benefits.

Our benefits department is always working to increase our number of accepted insurance plans. We want to be able to serve as many patients as we can. If your plan is not on this list, then please call Kyle Johnson at 217-744-8000 so that he can begin the enrollment process with your member services.
