Referring Physicians and Nurse Case Managers

At Premiere Physical Therapy, we appreciate referrals from physicians and nurse case managers. That’s why we have implemented procedures to make the referral process as quick and easy as possible.

If you are a physician or a nurse case manager who would like to refer a patient to our office, we ask that you follow these instructions:

  1. To refer a patient to our Downtown clinic, please call the reception staff, Coartney or Jennifer, at (217) 744-8000. To refer a patient to our West clinic, please call Nichole at (217) 793-0000. To refer a patient to our Virden clinic, please call Tony at (217) 965-5100. 
  2. Please have the following information available: the patient’s name and phone number, diagnosis, doctor’s name and patient’s type of insurance. We will let you know of available appointment times and make an appointment over the phone.

OR

  1. You may fax us the prescription at (217) 744-8004 at the Downtown clinic, (217) 793-5201 at the West clinic, or (217) 965-5151 at the Virden clinic. Please provide the patient’s home phone number. 
  2. To save your office staff time, we will contact the patient to obtain all necessary information to schedule an initial evaluation and verify all benefits prior to their arrival.
